What Causes Shoulder Pain? Common Shoulder Pain Generators Explained

Shoulder pain is one of the most common musculoskeletal complaints, but “shoulder pain” is not a diagnosis. The shoulder is a complex area with several joints, tendons, muscles, bursae, and nerves, and pain can come from any of them. The most common pain generators include the rotator cuff, bursa, AC joint, biceps tendon, labrum, frozen shoulder, and even the neck.


A very common cause is rotator cuff pain. The rotator cuff helps stabilize and move the shoulder. When those tendons become irritated or inflamed, patients often feel pain in the front or side of the shoulder, especially with overhead motion, reaching, lifting, or lying on that side at night.


Another common issue is bursitis. The bursa is a fluid-filled sac that reduces friction around the rotator cuff. When inflamed, it can create pain with motion and tenderness around the shoulder. Bursitis often overlaps with rotator cuff tendinitis and impingement rather than occurring in isolation.


Frozen shoulder, also called adhesive capsulitis, is different. It causes both pain and major stiffness. Patients often notice that not only does moving the shoulder hurt, but the shoulder simply will not move normally anymore. Both active and passive range of motion become limited, and recovery can take months AAOS.


AC joint pain is usually located at the top of the shoulder near the collarbone. It often hurts when reaching across the body, carrying heavy objects, or doing pressing movements. Biceps tendon pain is commonly felt in the front of the shoulder. Labral pathology may cause deep joint pain, clicking, catching, or instability.


Not all shoulder pain starts in the shoulder. A neck problem, such as cervical radiculopathy, can refer pain into the shoulder and down the arm. If symptoms include numbness, tingling, weakness, or pain extending below the elbow, the neck should be considered.

The best treatment starts with the right diagnosis. That may include a physical exam, imaging, rehabilitation, medications, or diagnostic injections depending on the suspected source of pain.
